This is the entrance to the Cave Adder cave. Adders also spawn outside the cave at the entrance. Cave adders are a level 35 single down-arrow mob that comes in groups of 4. These mob groups are bugged - some are inside the walls and under the earth. It is a pain but just stick with it and you will get your 20.
